To register for the Summer Institute, please return the registration form and submit your payment.
Registration deadline is one week prior to training start date.
Space is limited to 25 people per training.
No registration at the door.
- Download and complete one of the following identical registration forms

In Word. The registration is an electronic form. Save the form to your desktop, complete the form, save it again, and then email it to cnysel@contactsyracuse.org

In PDF. Open and print the PDF. Complete the form
and Fax it to 315-251-2218.
- Submit payment online or send a check to the address on the registration form

$79 per person for 4 or more people from the same organization
- Or $99 per person
